ABSTRACT
Supporting the transition from pediatric to adult oncology presents challenges for all stakeholders, including adolescent and young adult (AYA) patients, their parents, pediatric oncologists, and medical oncologists. In this article, we describe the different stages of this transition and the transfer of care, the obstacles to the transition, and potential solutions. We discuss the contribution of AYA care in oncology through collaborations between pediatric and medical oncologists, and the importance of multidisciplinary support for patient empowerment. We draw on the example of the SPIAJA team at Gustave Roussy, whose flexible transition process adapts to each young person and provides avenues for discussion about a structured transition program. “The transition from pediatrics to adult medicine must be planned, coordinated and supported,” as recommended in the 2009/2013 Cancer Plan.
